SAFETY PLAN
ACCIDENT PREVENTION PLAN


I. Company Safety Policy

It is the desire of FEI Enterprises, Inc. that all employees recognize the value of practicing and adhering to the safety program outlined herein. It is FEI Enterprises, Inc.'s intent to cooperate with all agencies concerned with safety programs in an effort to eliminate injuries to employees who come under our jurisdiction.

   

II. Management Organization

FEI Enterprises, Inc. – Company Safety Officer and Project Manager and Site Safety Officer/Superintendent FEI Project Manager and Superintendent have full responsibility and authority to establish and change any or all work methods and operations. By subcontract, the establishment of work methods is delegated to each of our subcontractors, and in turn to the superintendents. Strict adherence to the FEI safety plan will be required of each subcontractor.

III. Accident Prevention Responsibilities

A. Company Safety Officer and Site Safety Officer will be directly responsible for the safety program. All foremen will at all times be obligated to assist the Safety Officer in fulfilling the safety program.

B. The Project Manager will make periodic unscheduled safety inspections to ensure that all employees and subcontractors are operating in accordance with this safety plan.

C. Each new employee employed on the project will receive safety indoctrination. This will be given by one of the personnel named in item 3A above.

D. The Company Safety Officer or Site Safety Officer will give newly appointed supervisors additional safety instruction. This indoctrination will include, as a minimum:

1. Restatement of company safety policy and project commitment to safety.
2. Review location of first aid equipment, fire extinguisher, and other safety related equipment.
3. Review fire and accident reporting procedures.
4. Responsibility of supervisors regarding records and reporting of incidents/accidents to the Client/Client Representative.
5. Assignment of required safety equipment, manuals, and report forms.

E. Any employee or subcontract employee who willingly or unwillingly violates the terms of the safety plan as a matter of habit or otherwise, will be warned. If such violations continue after being given warnings, the employee will be terminated.

IV. Subcontract's Participation

A. Any subcontractors engaged in work under FEI Enterprises, Inc. contract will become familiar with the Accident Prevention Program and will be required to abide by all of the safety stipulations contained therein. They will be required to conform to the same contract requirements in performance of their work as the prime contractor.

B.  Subcontractors will hold weekly safety meetings with their own personnel.

C. Subcontractors will conduct the required pre-work safety indoctrination for their own employees.

V. Emergency Plans

A.  Emergency telephone numbers and reporting instructions for ambulance, hospital, fire and police shall be conspicuously posted at each project. These numbers and procedures will be on FEI safety bulletin boards and in the project office near the telephone.

VI. Accident Reporting

A. All accidents will be investigated and reported to the job site office.

B. All serious accidents (i.e. involving death, loss of limbs or over a 24-hour hospitalization) will be reported to CAL-OSHA. This report will be taken over the phone by calling CAL-OSHA.

C. All accidents will be reported to the home office immediately on a Supervisor's Accident Report form. The completed form will be faxed to the home office within 24 hours (Fax 213-937-0858).

VII. Pre-work Indoctrination and Training

A.  Pre-work Safety Indoctrination:
Each employee reporting for work will be given initial indoctrination and training. This initial indoctrination will include the following:

1. Statement of FEI Enterprises, Inc. Safety policy.
2. Review of general rule.
3. Brief description of project, work areas and job hazards.
4. Identification of key personnel and responsibilities thereof.
5. Accident reporting procedures.
6. First aid facility locations.
7. Workman's Compensation carrier identification. State Fund Insurance Company.

B. Jobsite Safety Meetings and Training:

1. Safety meeting will be held at least once each week.
2. The topic of training will be supplied as determined by the Safety Officers and Project Manager.
3. Other items of discussion to include anticipated work ahead and expected hazards, review of any recent accidents or near-miss events, questions or suggestions for employees.

C. Safety Bulletin Board:
In addition to project required bulletin boards, FEI maintains a special safety bulletin board, upon which is posted safety posters, project safety rules and any other items strictly regarding health and safety on the project.

VIII. Physical Qualification of Employees

A. Employees reporting to work are to be physically ready to work, including proper clothing, etc. Employees are assumed to be fit unless definite outward evidence indicates otherwise. The Project Superintendent may question employees regarding their health.

B. Any employee required to work in “high” areas will be questioned specifically in this regard, as to previous experience, etc.

IX. First Aid Facilities

A.  A first aid kit will be provided in FEI's jobsite storage bin with appropriate signs indicating its location. The first aid kit location will be covered in the employee pre-work safety indoctrination. In addition, FEI Enterprises, Inc.'s superintendents and foreman carry smaller first aid kits in their vehicles.

B.  Emergency medical service is available by calling 911

The above address will be given to employees during the indoctrination.

X. Sanitation

A. Jobsite drinking water will be provided in “cool-cans”, “igloos”, or similar containers. All drinking water will be imported “sparkletts” or similar. Paper drinking cups and waste receptacles for used cups will be provided.

B. Temporary toilet facilities may be available at the project site.

XI. Housekeeping

A. FEI Enterprises, Inc. Site Safety Officer is also responsible for housekeeping. He will delegate duties in this regard as necessary to maintain a safe and clean workplace. Subcontractors are individually responsible for maintaining their work areas.

B. Cans for disposal of trash will be provided. A commercial bin for collection of trash from cans will be utilized, if necessary. Oily rags, flammable and hazardous waste generated will not be disposed of in trash cans or bins. Approved containers for such material will be used.

XII. Material Storage

A.  Material deliveries whenever possible will be planned relative to available storage area.

B. Material required to be stored at the jobsite should be stored in such a manner as to be reasonably accessible relative to the manner in which it will be required to be moved. If items are to be taken from storage by hand, clear pathways to the material must be maintained. Materials shall be piled in such manner as to remain stable at all levels of usage.

C. Any flammable or combustible liquids shall be stored at least 50 feet away from combustible construction. “No Smoking!” signs shall be posted at these areas.

D. Paint and painting materials shall be stored in accordance with NFPA recommendations. MSDS will be available on-site for all hazardous materials used in conjunction with each building.

XII. Protective Equipment

A. Eye protection, either safety glass type with side shield or goggle type, will be provided for general use when necessary. This protection must be used in the following operations:

1.  Grinding or drilling metal
2. Grinding or drilling concrete, block, stucco or like construction
3. Operating electric or battery powered saws
4. Operating hilti or janerco gun

B. Appropriate darkened goggle must be used in all flame cutting operations and appropriate welding helmets shall be used by all welders.

C. Gloves, rubber boots, and raincoats will be provided if work requires their use.

D. Employees shall wear clothing suitable for weather and work conditions. The minimum shall be short sleeve shirts, long trousers and leather or other protective work shoes or boots. Canvas tennis or deck shoes are acceptable.

E. Burning permits will be obtained from the base fire department before any burning or welding is done.

XIV. Fire Protection

A. Fire extinguisher will be provided as follows:

1.  In the storage container for control of any fire therein.
2. In all FEI Enterprises, Inc. superintendents or foremen's vehicles.
3. Within 25 feet of any flammable storage area.
4. Fire extinguishers are dry chemical ABC type.

XV. Fire Prevention

A. Flammable liquids shall be stored in VII-C above. All containers used in storage and handling of flammable or combustible liquids shall be suppliers original labeled container or UL or FM listed containers or equipment.

B. Paint to be stored as per VII-D above. All paint to be stored separate from ordinary combustible.

C. Smoking will be allowed only when conditions permit such activities in designated areas.

D. Burning permits will be obtained from the base fire department before any burning or welding is done.

XVI. Ladders and Stairways

A. Ladders on this project shall conform to the latest edition of ANSI A14.1:A14.2:A14.3.

B.  All ladders shall be secured at the top to prevent movement while climbing.

C.  Broken or damaged ladders shall be removed from service immediately.

D. Provisions shall be made for landing at the top of all fixed ladders. Side rails or handhold extending 42” above landing shall be provided.

E. Portable metal ladders shall not be used for electrical work of where they may contact electrical conductors.

XVII. Construction Equipment

A. Construction Equipment – General

1. Before any piece of equipment is placed in use on the project it shall be inspected and tested by a competent mechanic and certified to be in safe operating condition.
2. The operator of each piece of equipment shall make daily inspection of his equipment. Tests shall be made prior to beginning work each day to determine that brakes and operating systems are in proper working condition.
3.
Any equipment found to be unsafe should be removed from service until repaired.
4. Only properly qualified operators shall operate equipment.
5. All belts, gears, pulleys, or other moving parts exposed to contact by persons shall be guarded by protection meeting the requirements of ANSI B 15.1.
6. Mechanized equipment shall be shut down prior to and during fueling operations.
7. All construction equipment except light duty trucks shall be equipped with a reverse signal alarm meeting the requirements of SAE J 994b.
8. Getting on or off equipment while it is in motion is prohibited.
9. Persons are not allowed to ride on a piece of equipment except if sitting in a seat or equally protected location. Seatbelts should also be used when operating motor vehicles.
 

B. Motor Vehicles.

1. Every person operating a motor vehicle shall possess at all times while operating such vehicle a valid operator's license for the vehicle being operated. All vehicles shall be operated in a safe and legal manner and at speeds within the posted speed limit. Special notice of posted speed limits in project areas shall be made.

XVIII. Electrical

A. Electrical Hazard Control

1. Personnel familiar with code requirements and qualified for the class of work to be performed shall do all electrical work.
2. Temporary wiring shall be guarded, buried, or isolated by elevation to prevent accidental contact by workers or equipment.
3.
A multiconductor cord having an identified ground conductor and multicontact polarized plug in receptacle shall ground portable and semi-portable electric tools and equipment. Plastic handled “double insulated” small power tools may be used if they are so indicated on the tool body by the manufacturer.
4. A ground shall be provided for non-current carrying metallic parts of generators, electrically powered arc welders, switches, fuse boxes, distribution cabinets, etc.
5. Switches and circuit breakers shall be marked, labeled or arranged for ready identification of circuits or equipment supplied through them.

a. Switches shall be of the enclosed safety type with enclosures grounded.
b. Switches or breakers shall be provided with a means for locking in the “off” position during maintenance periods.
c. Circuit breakers shall be of proper rating for the circuit protected.

6. Temporary power lines, switch boxes, receptacle boxes metal cabinets, shall be marked to indicate the maximum amperage and operating voltage.
7. Whenever possible, all electrical equipment and circuits to be worked on shall be de-energized, red-tagged and power “locked out” before work is started therein, and personnel shall be protected by clearance procedures and electrical isolation.
8. Flexible extension cords shall be of a type listed by the U.L. and intended for hard usage as specified in the NEC.

XIX. Poisonous and Harmful Substances

Except as indicated below, no exposure to harmful substances is anticipated on this project.

XX. Noise Control

A. If sound levels exceed the allowable exposure per Table 5-3-1 of EM 385-1-1 personnel protection will be provided and required for exposed employees. No exposure is anticipated at this time.

B. A supply of approved hearing protectors is available in the first aid kit. Employees with problems or otherwise desiring to utilize protection are given protectors and encouraged to use them.

XXI. Painting Operations

A. All painting operations shall be conducted in accordance with EM385-1-1.

1. No smoking will be allowed in the vicinity of any painting operations.
2. Paint scraping and paint saturated debris will be removed daily.
3. Packages containing paint thinners or other volatile painting materials shall be kept tightly closed when not in use and shall be stored per NFPA recommendations.
4. Drop cloths and appropriate masking techniques shall be employed to protect existing structures and surfaces

XXII. Hand and Power Tools

 A. Hand Tools

1. All hand tools shall be in good repair and shall be used only for the purpose for which they were designed. Tools having defects that impair their strength or render them unsafe shall be removed from service.
2. When work is being performed overhead, tools not in use shall be secured or placed in holders.
3. Throwing tools or material from one location to another is not permitted.
 

B. Power Saws

1. Power saws shall be inspected before use and determined to be in safe operating condition prior to use. Specific items to be checked include:

a. Blade sharp and properly installed
b. Switch operates smoothly
c. Bladeguard operates correctly
d. Power cord in good condition, no nicks to the wire, plug in good condition
e. Body and handle in good condition, no cracks or missing parts
f. Saw runs smoothly without excessive brush sparking; appears to develop full power

C. Grinders

1. Grinders shall be inspected in similar manner as listed for power saws above. The grinding disc shall not be excessively worked, shall be installed properly and have no discernible defects.
2. Grinding disc shall be used only for the material indicated. Discs not identifiable as to use by either color of back or by printing on the back shall not be used.
3. Eye protection is required in all grinding operations.

D. Pneumatic Tools

1. Pneumatic tools shall be inspected and determined to be in safe operating condition before use. Tools having body or handle cracks, missing or defective tool holders or retainers shall not be used.
2. Safety lashing is required at connections between tools and hose, and at all quick makeup type connections.
3. Compressed air shall not be used for blowing dirt from hands, face or clothing.
